   SCHOLARSHIP APPLICATION

   *Eligibility*

 

       Applicant must submit the following items:

​

1. Completed application forms (please print legibly or type).

2. Two (2) character references in the form of a letter of recommendation.

3. A copy of their most recent transcript with cumulative grade point average.

4. Personal Essay – Describing your background, personal accomplishments and challenges, education and career goals.

5. Applicant must agree to a personal interview with the selection committee via phone or in-person.
